Dear Parent/Guardian or student over the age of 16,
As of the date in the first sentence of the letter, the student’s vaccine record with Public Health is incomplete. The student requires vaccines for the diseases listed in the letter or a valid exemption. The vaccine information Region of Waterloo Public Health has available for your student is shown on page two of the letter you received.
To get the student's record up-to-date choose an option below:
- If this student has already received the vaccines listed on the first page of the letter, please report them to us.
- If this student needs a vaccine, visit your health care provider or visit a walk in clinic (call ahead) to get vaccinated and bring the letter with you. Once the student has had the vaccine, report it to Public Health.
- If this student does not have an Ontario Health Card (OHIP) or a health care provider, book an appointment with Public Health.
- If you do not wish to vaccinate this student, please visit our vaccine exemptions page to review your options.

Please update Public Health every time this student receives a vaccine. Your health care provider does not report vaccine records for you.
The student may be suspended from school for up to 20 days if we do not receive an up-to-date vaccination record or a valid exemption.
Deadlines to submit records are:
- April 6, 2026 for secondary (Grades 9 to 12)
- May 4, 2026 for elementary (Junior Kindergarten to Grade 8)
